MetLife Premium Statement

The Premium Statement should be completed on the first of each month in order to calculate the total premium due. To download one the Premium Statements, which are on Excel spreadsheets, right click on a link below, highlight "Save target as…" and save the document to your files.

To complete the form:

  • Enter your client name, policy number, classification, bill branch, and premium due date.
  • Press the enter key or the down arrow to proceed down the column to enter your information.
  • The program will automatically calculate the totals for you.
  • Don't forget to type your name and date in the "Prepared by" and "Date" spaces at the bottom of the page before saving and emailing your form.

To save completed forms:

  • Click File/Save As
  • When prompted, name the file. You might wish to include the date in the file name so it can be easily located should you need to refer to it at a later date.
  • Click the Save button.
  • For the following month's Premium Statement, save another copy of the prior month's statement under a new file name (changing to the new statement date, for instance) and update the information using the instructions for completing the form above. Click the Save button and submit as explained below.
